I want to start by drawing a clear line, because I think it gets blurred constantly online: what I described in my last post — finding a patient's effective dose within the standard, approved range — is a completely different conversation from what's usually meant by "microdosing" a GLP-1.
This post is about that second, separate conversation: doses below the standard approved starting dose. I don't love the term "microdosing" for this — it's borrowed from a different context and it undersells how seriously this needs to be approached clinically. But it's the term most patients ask me about, so I want to address it directly and honestly.
What's actually being asked here
When patients bring this up, they're usually not asking about weight loss at all. They're asking about something else entirely: whether very low, sub-therapeutic-for-weight-loss doses of GLP-1 medications might offer benefits independent of appetite suppression — things like anti-inflammatory effects, metabolic and cardiovascular signaling benefits, or other systemic effects that some early research suggests may occur at exposures well below what's needed to meaningfully suppress appetite.
That's a real and legitimate area of scientific interest. GLP-1 receptors aren't just in the gut and appetite centers — they're found in cardiac tissue, blood vessels, immune cells, and elsewhere, which is part of why researchers are looking at these medications for uses well beyond weight management. It's plausible that some of those effects could occur at lower exposures than what's needed for significant appetite suppression.
Where I want to be careful
Here's what I won't do: present this as an established, well-studied protocol with a defined dose and a defined benefit. It isn't that yet. Much of what's discussed under the "microdosing" label is extrapolated from receptor biology and early or preliminary research, not from large trials specifically testing very low doses for these purposes. That's an important distinction, and it's one that gets lost in a lot of the online conversation about this.
What I can say is that the underlying biological plausibility is real, the interest from serious researchers is real, and this is an area worth paying attention to. What I can't say — and wouldn't want to imply — is that there's a settled, proven protocol for it yet.
Why this is not a self-directed decision
Below-approved-dose use of any medication is, by definition, off-label territory, and it's a decision that needs to be made individually, with a provider who knows your full medical history, not something to approach based on what's circulating in online communities. The risk profile, monitoring needs, and whether it makes sense at all is going to be different for every patient — and for a lot of patients, it won't be the right fit at all.

What I'd want you to take from this
There's a real, biologically plausible reason researchers are interested in very low-dose GLP-1 effects beyond weight loss. That's worth knowing. But there's a meaningful gap between "biologically plausible and being studied" and "established clinical protocol," and I think patients deserve to hear that gap described honestly rather than have it papered over by a trend with a catchy name.
