Almost everything else on this site assumes you already know what "metabolic health" means. I don't think that's a fair assumption, so I want to back up and start here.
Metabolism, in plain terms
Your metabolism is the sum of every chemical process your body runs to keep you alive and functioning: converting food into usable energy, building and repairing tissue, regulating temperature, managing hormones, clearing waste. It's not one system. It's the coordinated output of many systems working together, running constantly, whether you're asleep, exercising, or sitting still.
When people say "metabolism" casually, they usually mean something narrower — how efficiently your body burns calories. That's part of it, but it's a small part. Metabolic health is really about whether all of these interconnected systems are regulating themselves correctly: whether your body is accurately sensing hunger and fullness, storing and releasing fat appropriately, keeping blood sugar in a healthy range, managing inflammation, and maintaining the hormonal signaling that coordinates all of it.
What "healthy" actually looks like
A metabolically healthy body does a few things well, mostly without you noticing:
- It regulates appetite accurately. Hunger and fullness signals correspond reasonably well to what your body actually needs.
- It manages blood sugar efficiently. Glucose gets moved out of the bloodstream and into cells for energy without requiring excessive insulin to do it.
- It stores fat appropriately, and in the right places. Some fat storage is normal and necessary. The problem isn't fat itself — it's where it accumulates and how much, which I'll get into in a future post on visceral fat specifically.
- It keeps inflammation in check. Low-grade, chronic inflammation is one of the quiet threads connecting almost every metabolic problem I treat.
- It maintains muscle. Muscle isn't just about strength — it's one of the most metabolically active tissues in your body, and it plays a direct role in blood sugar regulation and resting energy expenditure.
When these systems are working well together, weight tends to sit in a stable, healthy range without constant effort. That's an important point, and one I don't think gets said often enough: for a metabolically healthy person, weight management isn't usually a daily struggle. It's a byproduct of systems doing their job.
What happens when it breaks down
Metabolic dysfunction is what happens when one or more of these systems stops regulating itself correctly. It rarely announces itself with a single dramatic symptom. It tends to show up gradually — hunger signals that don't track need accurately, insulin that has to work harder to do the same job, fat that accumulates in metabolically dangerous locations, inflammation that stays elevated, muscle that quietly erodes over years.
None of this shows up clearly on a scale. That's part of why I've spent so much of this site pushing readers away from weight as the primary signal. Weight is a downstream consequence of these systems, not a direct measure of them. Two people can weigh the same amount and be in completely different metabolic positions.
This is also why terms like obesity, metabolic disease, metabolic syndrome, visceral fat, and muscle loss aren't separate, unrelated problems. They're different expressions of the same underlying issue: one or more of the regulatory systems I described above breaking down. Over the next several posts, I'm going to walk through each of these individually — what they actually mean medically, how they're diagnosed, and how they connect to each other. This post is meant to be the baseline you can measure all of that against.
Why I think this framing matters
Most public conversation about weight treats it as a standalone issue — something to fix directly, with diet or willpower or a prescription. Twenty-five years of treating this surgically, and more recently medically, convinced me that's the wrong frame. Weight is a symptom. Metabolic health is the actual condition underneath it, and it's the thing that determines whether someone is going to be functional, strong, and free of chronic disease burden decades from now.
If you take one thing from this post, I'd want it to be this: the goal was never to weigh less. The goal is a body whose regulatory systems are working correctly. Weight often follows from that. It was never supposed to be the target itself.
