I want to tell you about two patients, because I think their story makes a point more clearly than any explanation I could give on its own.
Two patients, the same number
A few years ago, I had two patients finish a year of treatment within a few weeks of each other. Both had lost exactly 40 pounds. On paper, in the way most people would describe their results, they'd had identical outcomes.
Their DEXA scans told a completely different story. The first patient had lost roughly 34 pounds of fat and 6 pounds of muscle — a ratio I've described elsewhere on this site as fairly typical without deliberate intervention. The second patient, who'd been doing resistance training and hitting a deliberate protein target throughout treatment, had lost 42 pounds of fat and actually gained 2 pounds of muscle. Same scale number. Genuinely different physiological outcomes, different metabolic trajectories, and — though it would take longer to show up — different risk of regaining weight down the road, for the reasons I described in an earlier post on why patients regain.
This is the whole argument of this post, distilled into one comparison: weight loss and fat loss are correlated, but they are not the same thing, and the gap between them matters more than almost anything else I've written about on this site.
Why the scale can't tell these two patients apart
The scale measures one number: total mass. It has no way of distinguishing what that mass is made of. Fat, muscle, water, even the contents of your digestive system at the moment you step on it — all of it registers as the same undifferentiated number. Two patients can lose an identical amount of weight through completely different physiological processes, and the scale will report them as equivalent successes.
This is why I've spent so much of this site pointing readers toward DEXA scans and body composition tracking instead of weight alone. It's not that weight is meaningless — it's a reasonable proxy, and it's certainly easier to measure than a DEXA scan. But a proxy is only useful to the degree it reflects the thing you actually care about, and in weight loss specifically, that proxy breaks down often enough that I don't think it deserves the primacy it's given.
The clearest illustration: patients whose weight barely moves
If the scale-versus-fat distinction still feels abstract, here's the version of it I find most convincing, because I've watched it happen repeatedly: patients who go through months of visible physical transformation — smaller waist, more defined muscle, clothes fitting differently — while their weight on the scale barely changes at all.
This happens when fat loss and muscle gain occur simultaneously, at roughly offsetting rates. The composition of the body is changing substantially. The total mass isn't. A patient in this situation, looking only at the scale, would reasonably conclude that nothing is working. A patient looking at their body composition would see a clearly positive trajectory. Both readings come from the same person, in the same timeframe. Only one of them reflects what's actually happening physiologically.
I've had to talk more than one patient out of abandoning a program that was genuinely working, simply because the scale wasn't moving the way they expected. That conversation — reframing what the scale can and can't tell you — has become one of the more common ones I have in follow-up visits.
Why this distinction matters beyond how you look
I want to be clear that this isn't just about aesthetics or a more satisfying number to report. The health implications diverge meaningfully depending on which kind of weight loss actually occurred.
Fat loss, particularly visceral fat loss, is associated with real improvements in insulin sensitivity, inflammatory markers, and cardiovascular risk — themes I've covered at length elsewhere on this site. Muscle loss, even when it happens alongside fat loss, moves in the opposite direction: it lowers resting metabolic rate, increases frailty risk over time, and — as I described in the post on why patients regain weight — sets up a less favorable trajectory for maintaining results long-term. A patient who loses 40 pounds that's mostly fat is in a fundamentally different, and better, position than a patient who loses 40 pounds with a significant muscle component, even though both would report identical success by the only number most people track.
What I'd want you to take from this
If there's a single idea I'd want to leave you with, across everything I've written on this site, it's this one: ask what kind of weight you're actually losing, not just how much. That question changes what "success" looks like, it changes what you should be tracking, and — based on two patients who once handed me the exact same number on a scale — it can mean the difference between two outcomes that look identical on paper and are anything but identical in the body underneath.
