For most of my career, this wasn't really a question. Bariatric surgery was the treatment with the strongest evidence, the most durable results, and the best long-term outcomes, and medication played a supporting role at best. That's changed. GLP-1 therapy now produces weight loss that, at the highest doses of the newest drugs, genuinely approaches surgical outcomes. I think the public conversation about this has settled into a "surgery versus drugs" framing that a recent comparative review gets right to push back on — and having spent 25 years doing the surgery and, increasingly, prescribing the medication, I want to walk through why I think that framing is already outdated.
The efficacy numbers, side by side
Semaglutide, in the trial that established its use for obesity, produced average weight loss of 14.9% at 68 weeks. Tirzepatide, in its pivotal trial, produced 15.0% to 20.9% depending on dose, over 72 weeks. A direct head-to-head trial found tirzepatide outperforms semaglutide by roughly 6% at matched obesity-indication doses. And the next generation is already here: retatrutide, a "triple agonist" targeting GLP-1, GIP, and glucagon receptors simultaneously, has reported a headline 28.7% mean weight loss at its highest dose in Phase III trials — a number that would have been unthinkable for a medication just a few years ago.
Bariatric surgery's numbers remain higher still, at least at the outset. A recent UK randomized trial found Roux-en-Y gastric bypass produced 26.8% weight loss, sleeve gastrectomy 19.4%, and gastric banding 14.0%. The gap between top-tier medication and top-tier surgery is real, but it's narrower than it's ever been, and it's narrowing further with each new drug approval.
Where surgery still has an advantage medication hasn't caught up to: durability
This is the part I think gets least attention in casual comparisons, and it's where my surgical background makes me want to slow down. The Swedish Obese Subjects study followed bariatric surgery patients for up to 20 years and found sustained weight loss throughout — 23% at 2 years, 17% at 10 years, 16% at 15 years, and 18% at 20 years. That's two decades of follow-up data, and it comes with reduced rates of heart attack, stroke, type 2 diabetes, cancer, and a genuine reduction in overall mortality.
GLP-1 trials simply don't have that yet. Most have followed patients for two to three years. And the discontinuation data I've covered in an earlier post on this site is sobering: in the STEP-1 extension, patients regained roughly two-thirds of their lost weight within a year of stopping semaglutide, landing at a net loss of only about 5.6% from baseline. Surgery's outcomes, once achieved, don't require ongoing medication adherence to persist. That's a genuinely different risk profile, and it's not a small consideration for a patient trying to decide between the two.
Where medication has a real advantage surgery doesn't: everything about the risk profile
I don't want this to read as a case for surgery over medication, because it isn't. Surgery carries real risks that medication does not, most obviously surgical complications, but these are typically rare and manageable. What's genuinely different is durability: once surgery's results are achieved, they don't require ongoing adherence to persist, whereas any complication that does arise isn't quite as simple to reverse as stopping a prescription would be. GLP-1 therapy's downsides — mostly gastrointestinal, occasionally more serious like pancreatitis — are meaningful but categorically different in both severity and reversibility.
What the comparative studies actually show, with real caveats
A retrospective study comparing surgery to GLP-1 therapy in patients with diabetes and obesity found surgery associated with a 32% lower risk of all-cause mortality. I want to be precise about a real limitation here: most of the GLP-1 patients in that comparison were on older medications at diabetes-management doses, not the newer, higher-dose formulations actually used for obesity treatment today. That's a meaningful caveat, not a footnote — comparing surgery to yesterday's GLP-1 regimens isn't the same as comparing it to today's.
A more recent network meta-analysis found something I think is genuinely important: while surgery outperformed pharmacotherapy overall, the tirzepatide-specific subgroup showed no significant difference in weight loss compared to surgery. That's a real signal that the newest medications may be closing the gap that older comparisons captured. The authors are honest that all of these comparisons remain indirect — no head-to-head randomized trial between modern GLP-1 therapy and surgery currently exists — but the direction of travel is clear enough to take seriously.
The cost picture, which nobody talks about enough
There's a real financial dimension here that I think deserves more attention in these conversations. Surgery costs more upfront but tends to be more cost-effective long-term, given how durable the outcomes are. GLP-1 therapy costs less initially but accumulates ongoing, potentially lifelong costs. One retrospective analysis found the pattern flip within two years: higher monthly costs for surgery patients in year one, higher monthly costs for GLP-1 patients from year two onward. Both approaches have been judged cost-effective by health authorities like the UK's NICE, but the shape of that cost curve is genuinely different, and it's worth a patient understanding which shape actually fits their situation and resources.
What I actually do with this in my own practice: dual therapy
Here's where I want to go further than the research itself currently goes, because I think the field is still catching up to what's actually happening in practice. I have a meaningful number of patients on what I think of as dual therapy — both bariatric surgery and GLP-1 medication, used together as a coordinated plan rather than as sequential, separate treatments. The research so far mostly frames GLP-1 as a pre-treatment strategy to help patients reach surgery in a better starting position. That's real and valuable. But in my practice, dual therapy often runs the other direction too: patients well past their surgical recovery, sometimes years out, restarting or continuing GLP-1 therapy to address a plateau, address gradual regain, or simply optimize results further than surgery alone achieved.
I've found this combination genuinely powerful — more powerful, in a meaningful number of patients, than either approach alone would have been. It makes intuitive sense given everything covered above: surgery provides the durable anatomical and hormonal foundation, and GLP-1 therapy provides an additional, adjustable layer of appetite and metabolic regulation on top of it. I think this coordinated approach, rather than a strict either/or choice, is where the field is actually heading, even though the comparative research hasn't fully caught up to describing it yet.
What I'd want you to take from this
The honest answer to "surgery or GLP-1" is that framing the choice as binary is itself the mistake. Surgery has unmatched durability and decades of outcome data behind it. Modern GLP-1 therapy has a meaningfully better risk profile and is closing the efficacy gap faster than anyone expected a few years ago. And increasingly, in real clinical practice, the most powerful answer isn't choosing one — it's using both, thoughtfully sequenced or combined, based on where a specific patient actually is in their own trajectory. That's not a compromise position. Based on what I've seen treating patients both ways for 25 years, it's often the best answer available.
