This post starts something different: a foundational layer that sits underneath all of it. Regardless of which specific approach applies to your situation, a small set of basic nutritional elements — iron, vitamin D, calcium, B12, thiamine, a well-chosen multivitamin, creatine, collagen peptides, and adequate high-quality protein — function as the actual substrate that determines whether any of the more specific interventions on this site can do their job well.
Why I'm starting here
I originally thought about this pillar narrowly, through the lens of bariatric surgery specifically, since surgical patients have historically been the population most rigorously studied for nutritional deficiency risk, and there's real, well-established clinical knowledge there. But I don't think that framing is broad enough anymore. GLP-1 therapy has become, for many patients, the primary path to significant weight loss — and the nutritional foundation underneath that path deserves exactly the same attention, even though the underlying mechanism (reduced intake through appetite suppression, rather than altered anatomy) is different from surgery. The specific risks vary by approach, but the underlying principle doesn't: whatever changes your body is going through, it still needs the same basic raw materials to do it well.
The same foundation, showing up differently depending on your goal
I want to walk through this at a high level, because I think it's genuinely useful to see how the same short list of nutritional basics shows up across very different situations, before I go deep on any single one in the posts that follow.
If your goal is weight loss — whether through medication, surgery, or both — reduced food volume is itself a real risk factor for inadequate micronutrient intake, independent of whether anything about your digestive anatomy has changed. If your goal is maintaining results long-term, the nutritional demands don't relax just because the active weight-loss phase is over. Protein needs and micronutrient monitoring remain relevant for as long as your body composition is meaningfully different from where it started.
If you're recovering from an injury or focused on tissue repair, protein intake and specific cofactor nutrients — including some I'll cover in this pillar — become directly relevant to how well that repair process actually proceeds. Before any conversation about a specific medication, peptide, or surgical approach, there's a more basic question worth asking: is the foundation underneath that treatment actually solid. A sophisticated treatment plan built on top of an unaddressed nutritional gap is working with one hand behind its back, regardless of how well-designed the plan itself is.
