Why iron deficiency deserves real attention
Iron's most familiar role is in hemoglobin, carrying oxygen through the blood. But iron is also a required cofactor for a range of other processes, including energy metabolism and, as I'll get into below, healthy hair follicle cycling. Deficiency is common enough that it's worth taking seriously as a default consideration, not an edge case — menstruating women, anyone with reduced dietary intake, and patients experiencing absorption changes are all at elevated risk.
This connects directly to weight loss specifically. Appetite suppression from GLP-1 therapy genuinely reduces how much food most patients eat, and reduced food volume is itself a real risk factor for inadequate iron intake — independent of whether surgery has changed anything about digestive anatomy. This is the same principle I described in the opening post of this pillar: reduced intake creates real nutritional risk, regardless of which specific path led to it.
The consequence I want to focus on: hair loss, and why ferritin is the marker that matters
This is the part I think deserves the most direct, practical attention, because it's something I hear about constantly from patients going through significant, relatively rapid weight loss, and because the actual explanation is more specific than most people realize.
The type of hair loss most clearly tied to iron status is called telogen effluvium — diffuse shedding across the scalp, distinct from more localized patterns of hair loss. It happens when a physiological stressor pushes an unusually large number of hair follicles out of their active growth phase and into a resting, shedding phase all at once. Significant weight loss — whether from surgery, GLP-1 therapy, or aggressive dieting — is a well-recognized trigger for exactly this kind of stress response, because the body registers rapid, significant physiological change as a real stressor, regardless of how intentional or beneficial that change actually is.
Here's a detail I think is genuinely useful for patients to know directly: there's typically a real lag between the triggering event and the visible shedding, commonly six to eight weeks, with shedding often peaking three to six months after the trigger. That gap means a patient noticing hair loss well into their weight-loss journey may not immediately connect it back to changes that started months earlier — which is exactly the kind of disconnect I think this information should help close.
Ferritin, not simply "iron" broadly, is the specific marker worth checking. Ferritin reflects your body's stored iron reserves, which is a genuinely different measurement than hemoglobin or serum iron — it's entirely possible to have a normal hemoglobin level, with no anemia at all, while ferritin is already low. This is sometimes called nonanemic iron deficiency, and it's precisely the pattern most consistently linked to hair shedding in the dermatology literature. Multiple studies comparing women with this type of hair loss to unaffected controls have found significantly lower ferritin levels in the group experiencing shedding.
I want to flag something specific and, I think, genuinely useful: standard lab reference ranges for ferritin are not the right target for hair health. Many labs list the lower bound of "normal" around 12 to 15 ng/mL — but that threshold reflects the cutoff for diagnosing outright iron deficiency anemia, not the level needed to support healthy hair follicle cycling. Dermatology literature consistently associates ferritin levels below 30 to 40 ng/mL with increased shedding risk, and many practitioners specifically treating hair loss target a ferritin level around 70 ng/mL or higher as the actual goal — well above the point where a standard lab report would simply say "normal" and move on.
What I'd want you to take from this
If you're going through significant weight loss and notice increased hair shedding, I'd want you to know two specific things. First, don't assume a "normal" ferritin result on a standard lab report necessarily rules out iron as a contributing factor — ask specifically what the actual number was, not just whether it fell inside the reference range. Second, be patient with any correction: because of the lag built into the hair cycle itself, improvement in ferritin levels doesn't translate into visibly reduced shedding overnight; the literature suggests real improvement typically over six to twelve months once the underlying deficiency is actually corrected. This is a genuinely well-established, evidence-backed connection — not a wellness-space overstatement — and it's exactly the kind of basic nutritional foundation that deserves attention alongside whatever specific weight-loss approach you're using.
